For Eligible Medicare-Medicaid INTEGRITY Members
If you choose, you’ll have a care manager to help coordinate your care. The Neighborhood INTEGRITY care managers coordinate the care team. Your care manager will ensure your medical, behavioral health, substance use, long-term services and supports (LTSS), social and transportation needs are coordinated. Your care manager will coordinate these services and include them in your care plan.
Your care manager will also:
- Provide education about conditions, medications, treatment and resources in the community.
- Provide education on available benefits and services, medication adherence, self-management and goal setting.
- Coordination and facilitation of medical services and assistance in adherence to the member’s plan of care with their provider.
- Provide assistance with access to specialists and durable medical equipment (DME) providers. Enhancing communication between multiple providers that may be involved in the individual’s care.
What is a care manager?
A care manager is a qualified individual who helps you manage all of your providers and services. He or she works with your care team to make sure you get the care you need.
