Practitioners who leave our network but wish to continue to treat enrolled members under their care may submit a Continuity of Care Authorization Request to the Medical Management Department. Examples of situations that may fall within this policy include: care for a member who is pregnant, care for a member who is receiving services for an ongoing acute condition or an acute episode of a chronic illness, or providing specific services to a member when similar services are not available in our network.
